If you do not have insurance, please contact us for out-of-pocket rates. If one of our providers is Out of Network (does not take your insurance), than we will give you a form to fax to your insurance company for possible reimbursement. Reimbursement eligibility vary by insurance plan. Please contact your insurance company for further information.

Cancellation Policy

If you fail to give at least 24 hours advance notice of a cancellation or do not show up to your scheduled appointment, you can be charged a $50 no-show fee, due before your next appointment.
